Elders in Action expands Healthy Changes™ program across the state!
Improving Diabetes Education and Support for Older Adults
Healthy Changes™ is an evidence-based program designed to assist older adults in the daily management of their diabetes. Groups are led by trained Peer Leaders and focus on improving nutrition and physical activity. Meetings last 90 minutes and include concrete steps and information to help you increase your confidence in managing your diabetes, improve communication with your health care providers, and access more resources.
