Healthy Changes™
Improving Diabetes Education and Support for Seniors
Are you 55 or older and living with diabetes?
Would you like to:
- Increase your confidence in managing diabetes?
- Improve your nutrition knowledge and eating habits?
- Increase your physical activity?
- Improve communication with your health care providers?
- Know how to access more resources?
- AND, help others do the same?
Healthy Changes™ is an evidence-based program designed to assist older adults in the daily management of their diabetes. Groups are facilitated by Peer Leaders who have been specially trained in curriculum, problem solving and group leadership. The two-day training is led by Providence Health System, Elders in Action, and current Peer Leaders. It is free to participants.
Peer Leader Qualification:
- 55+ years old;
- First hand knowledge about and experience with diabetes;
- Willingness and ability to devote 3 to 5 hours per week volunteer time;
- Facilitation and group leadership experience or a willingness to learn skills;
- Good communication skills and a sense of humor!
- Trained Peer Leaders will work together with Elders in Action and a sponsoring organization to start new Healthy Changes™ groups.
